WELCOME to the WATTS COMMUNITY IMPACT  grant initiative

GRANT PROGRAM GUIDELINES: 

1. Applications are by invitation only.  
2. The application period is open from Friday, April 12, 2024 through Friday, May 3, 2024. 
3. Awarded grants will be active July 1, 2024 through June 30, 2025. 

 SELECTION CRITERIA PROPOSALS will be reviewed by a committee composed of Kaiser Permanente and Community Partners staff. Priority will be given to proposals that meet the following criteria: 

• Organizations located in and/or primarily serving the Watts community. 
• Organizations with a demonstrated commitment to advancing community health. 
• Organizations who have previously faced barriers to funding opportunities.  

 GRANT REQUIREMENTS: Organizations are NOT required to be 501(c)(3) organizations but are required to: 

• Submit financial documentation including tax returns and an IRS determination letter
• Sign a grant agreement electronically 
• Complete and submit an IRS Form W-9 and provide ACH bank transfer information 
• If applicable, provide a signed IRS tax return or Form 990 for 2021 and 2022 
• Provide other documentation or verification as requested

 SUBMISSION INSTRUCTIONS: 

• To submit an application, you must first create a free, Submittable account: https://manager.submittable.com/signup. Submittable is Community Partners’ online application portal. 
• As you fill out the online application, you will be able to save and return to it later.  
• The email address associated with your account will be used to communicate about your application. Please be sure to use an email account that is readily accessible and that you check often. Submittable should be used on Chrome, FireFox or Safari (Internet Explorer is not supported). 
• Community Partners strongly recommends submitting the application at least 5 days before the deadline to give ample time to troubleshoot technical issues.
