Thank you for your partnership in the Los Angeles County Department of Mental Health Transforming LA (TLA) program. We appreciate your work in supporting organizations in the TLA cohort with a project that strengthens the organization’s capacity to continue its prevention-related work. 

For your final report, please complete the following questions and upload project deliverables associated with this agreement. Additionally, please upload a brief narrative (suggested 3-5 pages) explaining how the finished project will support the organization's sustainability, particularly its ability to continue its work in providing prevention-based services that support mental wellness for communities across LA County. 

This final report is due with your final invoice. Though invoices are due on the 5th of the month, please let us know if more time is needed and we are happy to work with you to find a date that can work for everyone.
